I need help on Form Submission Dashboard (List Submissions): It only show submitted on, IP address and visitor ID. The rest of the parameters are not there such as name etc.

To display the other parameters of your form such as name, email address, etc. in the Form Submission Dashboard (List Submissions), you need to adjust the "Form Data Settings" under the "Settings" tab of your form.

Currently, only a few fields are selected for visibility in the List submissions that's why only those fields are shown in the List Submissions. If you want the rest of the parameters of the form will be shown in the list submissions, then please select the other fields from the "Column Visibility" option to display them in the List Submissions.
