I need to create a questionnaire with 5 surveys, and I need to get a separate score for each survey
and display all these results at the end of the overall survey. I want to clarify that I don't need to create 5 separate forms, but one form where the client takes one survey after another. Is this possible?

Yes you can create 1 form that contains different sections for different surveys. You can also use a Multi-page layout, use heading for different surveys.
