Does this form allow a workflow like the following?
Client fills out form with data on themselves. Client will also enter their doctors email and sign that the information is correct. Email will then be sent to the doctors email they provided which they will confirm all the information is correct and sign themselves.

As of now, you can do this:
Client fills out form with data on themselves, enter their doctor's email & sign. All information will be emailed to the doctor, optionally you can also enable email CC to be sent to the client. Email can include the PDF
Further, the doctor can check the PDF, and if all looks good then he can email to the customer.
